The basic methods that have been used for describing bound-state quantum electrodynamics are described and critically discussed. These include the external field approximation, the quasi-potential approaches, the effective potential approach, the Bethe–Salpeter method, and the three-dimensional equations of Lepage and other workers. Other methods less frequently used but of some intrinsic interest such as applications of the Duffin–Kemmer equation are also described. A comparison of the strengths and shortcomings of these various approaches is included.

BackgroundThe boundaries between health-related research and practice have become blurred as initiatives traditionally considered to be practice increasingly use the same methodology as research. Further, the application of different ethical requirements based on this distinction raises concerns because many initiatives commonly labelled as “non-research” are associated with risks to patients, participants, and other stakeholders, yet may not be subject to any ethical oversight. Accordingly, we sought to develop a tool to facilitate the systematic identification of risks to human participants and (...) determination of risk level across a broad range of projects and health-related contexts. This paper describes the development of the Public Health Ontario Risk Screening Tool.MethodDevelopment of the PHO Risk Screening Tool included: preparation of a draft risk tool ; expert appraisal; internal stakeholder validation; external validation; pilot testing and evalution of the draft tool; and revision after 1 year of testing.ResultsA risk screening tool was generated consisting of 20 items organized into five risk domains: Sensitivity; Participant Selection, Recruitment and Consent; Data/Sample Collection; Identifiability and Privacy Risk; and Commercial Interests. The PHO Risk Screening Tool is an electronic tool, designed to identify potential project-associated risks to participants and communities and to determine what level of ethics review is required, if any. The tool features an easy to use checklist format that generates a risk score associated with a suggested level of ethics review once all items have been completed. The final score is based on a threshold approach to ensure that the final score represents the highest level of risk identified in any of the domains of the tool.ConclusionsThe PHO Risk Screening Tool offers a practical solution to the problem of how to maintain accountability and appropriate risk oversight that transcends the boundaries of research and practice. We hope that the PHO Risk Screening Tool will prove useful in minimizing the problems of over and under protection across a wide range of disciplines and jurisdictions. (shrink)

Most jurisdictions require that a mental capacity assessment be conducted using a functional model whose definition includes several abilities. In England and Wales and in increasing number of countries, the law requires a person be able to understand, to retain, to use or weigh relevant information and to communicate one’s decision. But interpreting and applying broad and vague criteria, such as the ability ‘to use or weigh’ to a diverse range of presentations is challenging. By examining actual court judgements of (...) capacity, we previously developed a descriptive typology of justifications used in the application of the Mental Capacity Act criteria. We here critically optimise this typology by showing how clear definitions—and thus boundaries—between the criteria can be achieved if the ‘understanding’ criterion is used narrowly and the multiple rationales that fall under the ability to ‘use or weigh’ are specifically enumerated in practice. Such a typology-aided practice, in theory, could make functional capacity assessments more transparent, accountable, reliable and valid. It may also help to create targeted supports for decision making by the vulnerable. We also discuss how the typology could evolve legally and scientifically, and how it lays the groundwork for clinical research on the abilities enumerated by the MCA. No data are available. The relation between morality and religion has often been discussed. However, it is not always recognized that the relation varies greatly according to the variety of religions. I shall here be concerned solely with Christian theism in its traditional form. I take the latter to signify, essentially, belief in a morally perfect Creator who exists in the threefold form of Father, Son and Holy Spirit and who, in the person of the Son, became man in Christ for our salvation. I (...) thus exclude from consideration all non-theistic accounts of God or the Absolute. Also I shall consider, not simply bare theism of the kind that Christians share with Jews and Muslims, but also the distinctively Christian form of theism that is generated by distinctively Christian revelation. Many otherwise sound descriptions of the relation between morality and theism are defective because they fail to consider the distinctively Christian contribution to the theistic concept of God and of his relation to the world. Many writers often generalise about mysticism without a sufficiently close analysis of texts. Consequently the generalisations are often invalid. My present aim is to analyse one text and, in the light of this analysis, to offer some observations concerning mysticism in general and Christian mysticism in particular.

For over thirty years C. A. Campbell has made major contributions to both ethics and metaphysics. Since these do not correspond to the prevailing fashions in philosophy and theology they are in danger of being under-estimated, if not ignored. I hope to summarise and comment on them as impartially as possible. Inevitably I must be selective. In writing for this journal I have, naturally, chosen to stress those elements in Campbell's thought which are directly or indirectly relevant to religion. Even (...) so, there are many points which I have no space to develop. I shall be content if I say enough to indicate the importance of Campbell's writings for the study of the philosophically crucial topics to which they are devoted. (shrink)

Christianity affirms, with Judaism and Islam, that God is the omnipotent Creator of all things. But it diverges from them in also affirming that the Creator assumed a human nature in one figure of history, Jesus of Nazareth. Christ thus differs from other men in kind, not merely in degree; he is absolutely, not just relatively, unique. Admittedly many Christian theologians have held that the difference between Christ and other men is only one of degree. Yet the Church's traditional claim, (...) as expressed in the Chalcedonian Definition, is that Jesus was both creature and Creator, both fully man and fully God. (shrink)

High stress despite low tidal volumes may worsen lung injury and increase risk of death. No widely available parameter exists to assess global lung stress. We aimed to determine whether the volume delivered during a recruitment maneuver is inversely associated with lung stress and mortality in acute respiratory distress syndrome. Design: Substudy of an acute respiratory distress (...) syndrome clinical trial on esophageal pressure-guided positive end-expiratory pressure titration. Setting: U.S. academic medical center. Patients: Forty-two patients with acute respiratory distress syndrome in whom airflow, airway pressure, and esophageal pressure were recorded during the recruitment maneuver. Interventions: A single recruitment maneuver was performed before initiating protocol-directed ventilator management. Recruitment maneuvers consisted of a 30-second breath hold at 40 cm H2O airway pressure under heavy sedation or paralysis. VRM was calculated by integrating the flow-time waveform during the maneuver. End-inspiratory stress was defined as the transpulmonary pressure during end-inspiratory pause of a tidal breath and tidal stress as the transpulmonary pressure difference between end-inspiratory and end-expiratory pauses. Measurements and Main Results: VRM ranged between 7.4 and 34.7 mL/kg predicted body weight. Lower VRM predicted high end-inspiratory and tidal lung stress. After adjusting for Pao2/Fio2 and either driving pressure, tidal volume, or plateau pressure and positive end-expiratory pressure, VRM remained independently associated with both end-inspiratory and tidal stress. In unadjusted analysis, low VRM predicted increased risk of death. VRM remained significantly associated with mortality after adjusting for study arm. Conclusions: Low VRM independently predicts high lung stress and may predict risk of death in patients with acute respiratory distress syndrome. (shrink)

Research is ongoing concerning the medical risks and benefits of routine neonatal male circumcision. Interpretation of the data, however, is controversial. Circumcision presents a number of challenges in obtaining informed consent because of a variety of interesting issues. It is a nonemergent, elective procedure that allows for evaluation and discussion over time. In this respect, observation of the informed consent process for circumcision is relevant to other medical decisions, such as advance directives, discussed between primary care providers and patients. Obtaining (...) informed consent for circumcision has its constraints. The process generally occurs over a brief hospital stay. There have been recent changes in the official position regarding the procedure by professional medical societies, and it raises considerable psychologic issues for the provider and may Impact the way in which Information is provided.
